The 3am calf cramp starts with how you lie in bed
A randomized trial cut night cramps with a few minutes of stretching right before sleep. Here is how to do it tonight.
At 3am your calf locks into a cramp hard enough to throw you awake. A few minutes of stretching right before sleep cut how often that happened.
Night cramps get more common after 50, and the way you lie in bed sets them up. Your feet tend to point down for hours, which leaves the calf sitting in its shortest position all night.
A muscle that contracts while it is already short is the easiest one to tip into a cramp. The nerve endings driving it start firing on their own, and the muscle cannot let go.
Stretching just before sleep lengthens the calf and hamstring right before they settle into that shortened position. The likely explanation is that a freshly stretched muscle is harder to tip into that runaway firing.
A single cramp wakes you, and the calf can stay sore into the morning. Fewer cramps likely means fewer of those wake-ups, which is the part your energy notices the next day.
The trial, published in the Journal of Physiotherapy in 2012, randomly split 80 adults over 55 who had night cramps into two groups. One group stretched their calves and hamstrings every night immediately before going to sleep for six weeks, and the other did no stretching.
By the end, the stretching group was having 1.2 fewer cramps per night than the group that did nothing. The cramps that still came were 1.3 points less painful on a 10-point scale.
The comparison group did nothing, not a fake routine, so expectation may explain part of the effect. Still, a few minutes of stretching is a cheap trade for fewer 3am wake-ups.
Then sit on the edge of the bed with one leg straight along the mattress and fold forward from the hips until the back of the thigh pulls. Same count, same three rounds, both legs, then lie down.
The trial’s routine was brief and nightly, so doing it every night beats doing it hard. Stretch to tension, never to pain.
The cramp at 3am starts with a calf that spent hours short. A few minutes of length before sleep is how you change that.
If cramps are new, frequent, or come with numbness, weakness, or swelling, see a doctor, since some medicines and conditions cause them. Skip quinine and tonic water for cramps, because quinine is no longer recommended for this and can cause serious side effects.
